I created a possible draft of the SA Form 221 - Personnel Evaluation. My hope is that this form is simple, and useful for writers who would like to use it. I tried to mimic the general layout of the Request for Ship form, as well as the Request for Promotion form. I also included a Guidance section in the wiki article to help people fill it out.

I'd love to make sure this document is useful, above all else, so I'd welcome feedback on problems the form may have, or ways to make it better.

Purpose of Evaluation like a 4856, so you can say it's a periodic review, correctional action review, etc.

Implemented that fix, thank you! The Army counseling form was also a source I looked at to get ideas for what the form might look like.
